First Appointment 

What to bring on your First Appointment?

• Please bring your GP referral with you (if you haven't already faxed or emailed us).  Please note that GP referral's to specialists are valid for 12 months. 

• Medicare Card

• Health Fund Card ( No Gap Procedures ) 

• Pension Card (Dr Sidhu does bulk bill aged care pension card holders for Consultations)

• DSP ( is offered at a reduced rate for consultation)

• Veterans Affairs Card (DVA)

• Any test results that is relevant to the consult such as blood test, scan results, any previous investigations and medication list.
